CORE Application Team Meeting
Meets every Tuesday 8:30-10:00 a.m.

2. Work Session – April 21:
Stacey indicated that there was some confusion regarding the continued use of EMS as noted on Kathy’s Work Session Notes under Miscellaneous: “Will EMS still be used …” Stacey stated that as far as we know, EMS will still be used for scheduling. Facility information will still need to be entered in Colleague, and probably exported to EMS in a manner similar to the current TIP data transfer.
3. Finalize Recommendations: Protocol for changing name and address on Colleague
Kelley presented our examples to the Student Services Directors at their last meeting; they had nothing to add or any other suggestions. So CORE can move forward to establish appropriate protocols.
Student Name or Address Changes (Non-employee)
Discussion:
- Brenda: Current practice = looking up address in the phone book for every application received. If not in the phonebook, or if the student is applying in person, a copy of their driver’s license is collected.
- Terri: As of now, no address changes are taken over the phone (FERPA).
- Returned mail: When mail is returned for a student, should an address be changed or a “hold” placed on the record. Now that MCC’s envelopes are marked for “Return Service Requested”, mail is not forwarded and an alternate address is not noted on the envelope.
- When addresses are obtained or changed over the Web, will the student’s PIN (or other password/ID) be accepted as an electronic signature. Most in attendance agreed that the PIN would suffice, especially in Web Advisor as Perry explained. Perry also indicated that addresses changed over the Web are not permanently posted to the student’s Colleague account and are collected in a separate address field requiring verification.
- Beda asked if all signatures must be kept on file. Should the signature be placed in the student’s academic file or kept in a separate “address change” file by academic year; allowing for easy disposal after the retention period.
- Documentation requirements: There was a great deal of discussion on this issue including what documents will be accepted and appropriate verification processes.
- Document imaging capabilities: This is not included in Datatel, but Perry said there are some companies that Datatel works with on this. This may be a possibility in the future.
- Allowing numerous offices to distribute and collect the Change Forms would provide better customer service. These offices would then forward the form to the appropriate office (Records or HR) for processing.
Recommendations:
- Kelley suggested that a non-employee student name or address be changed bydesignated Admissions Office and Registration/Records Office staff only, as discussed previously.
- Name/Address Change Form: Terri said Jean is developing a Name/Address Change Form for Records Office use that should meet all of the needs we’ve mentioned, for both address and name changes. The form will require the student’s signature, and include a record of the required verification document(s). Kelley will meet with Jean and Terri to review the form.
- Address changes may be submitted:
- In person – with ID, Change Form with signature and required documentation
- Fax – Change Form with signature and required documentation
- Web – electronically signed Change Form (with Web Advisor PIN), to be verified with documentation
- Mailed in – Change Form with signature and required documentation
- Name Change required documentation: driver’s license, Social Security card, or legal court documents
- Name changes may be submitted:
§ In person – with ID, Change Form with signature and required documentation
§ Fax – Change Form with signature and required documentation
§ Mailed in – Change Form with signature and required documentation
- For consistent processing and easy reference, Patti suggested the Change Form should be required for all name andaddress changes for any person (student or employee), no matter how the change is submitted.
Faculty/Staff Name and Address (All employees, including students)
Discussion:
- Any employee name or address change will be done by Human Resources Office staff only.
- Address changes may be submitted:
- In person – with ID, Change Form with signature and required documentation
- Fax – Change Form with signature and required documentation
- Web – electronically signed Change Form (with Web Advisor PIN), to be verified with documentation
- Mailed in – Change Form with signature and required documentation
- Name Change required documentation: Social Security card or other legal court documents
- Name changes may be submitted:

- All required documentation is kept in an employee’s permanent file.
- As for students, Patti suggested the Change Form should be required for all name andaddress changes for any person (student or employee), no matter how the change is submitted.
4. Protocol for dealing with deceased person’s records: Kelley asked about current procedures.
Terri: A copy of the death certificate (can be unofficial) or obituary is accepted. Form can be processed by designated Admissions Office or Registration/Records Office staff only.
Mary: For any employee with benefits, an official copy of the death certificate is required.
Kelley: asked Mary and Terri to research wording for this procedure. Mary can get examples used by insurance benefit companies, and Terri will check for student insurance procedures. It was noted that there probably are insurance requirements for foreign students, and possibly athletes.

4 External Employment - Kathy met with Nancy Maycroft (internships, Co-op) and Carmella Martinez (work study students). Both were very impressed and excited about using this element in Colleague. Nancy uses several shadow systems now and performs tasks manually, as does Carmella, so this option would benefit them a great deal. Kathy will get notes to Kelley regarding their projected usage. 8 Bad Phone Number Tracking – Brenda asked if there is a way to track “bad” and former phone numbers in Colleague. (PENDING)
9 Veteran Types – Kelley, Kathy and Peggy (PENDING)
a) Kathy met with Lois Smith, Cashier, regarding her use of veteran/military information relating to authorization and student billing. This will be very beneficial for her and potentially alleviate a lot of paper sharing. The codes and programs we discussed look fine to her, as long as they agree with Financial Aid and Carmella.
b) Kelley will check with John Morano regarding veterans, and Peggy is checking the Financial Aid aspect.
c) Pat asked about “Veteran Programs”. This field is listed separate from “Veteran Types” for mapping processes. Kelley checked the CORE coding decision lists and didn’t see anything separate for “Veteran Programs”. Kathy had examples of the MINF (military information) screen and indicated that a free form text “Veteran Program” field is located after the “Veteran Type” field. Pat and Perry felt that some of this information may be outdated and they will check out this discrepancy.

Kathy, Terri and Darlene will not be able to attend next week’s CORE meeting due to all day SAM training. Kathy will get a tape recorder for Kelley to use so she can transcribe the minutes later.
